Cornerstone User Information

Agency Profile Update Form  

The Agency Profile Update form is used to update agency/site information, including address, telephone numbers, business hours and holiday schedule as well as personnel contact information for the use of Cornerstone support staff

If you need assistance in completing or submitting a Cornerstone form, please review the Completing and Submitting Cornerstone Forms page.

After completing the Agency Profile Update form, use the "Submit by E-mail" button to email the form to cstonesupport@cquest.us, or print and fax it to the Cornerstone Call Center, Attn: Support Database Specialist, at (217) 541-7475. A staff member from the Call Center will contact the agency with any questions; otherwise, the mailing list and databases will be updated accordingly.

Alternatively, you may report changes in Cornerstone system contact personnel information by calling the Cornerstone Call Center at toll-free 1-877-447-4221.
