Cornerstone User Information
Equipment Theft / Loss Report Form
The Cornerstone Equipment Theft/Loss Report Form is to be used to report Cornerstone equipment that has been stolen, lost, or damaged.
To report stolen, lost, or damaged Cornerstone equipment, complete the Cornerstone Equipment Theft/Loss Report, attach a copy of the police report (if applicable), and mail all materials to:

NOTE: To expedite this process, you may fax all materials to Pat Tribbet at (217) 492-5666 or contact the Cornerstone Call Center (877-447-4221), but you will still need to mail in the original form.
