Cornerstone User Information

Security Access Request Forms

To access the Cornerstone Security Access Request Forms, you must have a User ID and password. This information was sent to each Cornerstone site via BOD messages. If you are unsure of the User ID and password, please contact the Cornerstone Call Center at toll-free 1-877-447-4221. Upon verification of your Agency ID number, Call Center staff will supply you with the correct User ID and password.

If you know the User ID and password, you may access the forms. Once you have entered the User ID and password, you will have access to all the forms until your browser session is terminated.

The Request for Removal of Access Rights Form (October 2001) should be used to remove special access to Cornerstone, including Total Access for WIC, Cornerstone Administrative Rights.

PLEASE NOTE:
If you have trouble accessing or viewing any of these PDFs, please complete the following steps:

  1. Verify you have Adobe Acrobat Reader version 5.0 or higher. If you have a lower version, or do not have Adobe Acrobat Reader at all, you can download it for free at http://www.adobe.com/products/acrobat/readstep.html.
  2. Right-click on the link to the desired form, and choose "Save Target As" or "Save As".
  3. Save the form (PDF) to your desktop.
  4. From your "Start" button, access "Programs" and Adobe Acrobat Reader.
  5. In Reader, select File > Open.
  6. Navigate to the desktop, and choose the PDF you just saved. Click "Open".
  7. The PDF should be displayed correctly.

If, after completing these instructions, you still cannot access or view the PDF correctly, please contact the Cornerstone Call Center at toll-free 1-877-447-4221, and a call taker will be happy to fax you the forms.


[get Acrobat]In order to successfully view many documents and forms (PDFs) on this web site, you must have the latest version of Adobe Acrobat Reader. To download or upgrade this software for free, click on the Adobe icon to the left.

TIPS: If you have problems printing a PDF, try clicking on the "Print as Image" box in the Print pop-up box. If you have problems viewing a PDF, right-click on the link, select "Save Target As...", and save the PDF to your desktop. Then open Adobe Acrobat Reader (from your Start menu), choose File - Open, and select the recently saved PDF from your desktop.
