Cornerstone User Information

Completing and Submitting Cornerstone Forms

IPHCA / CQuest will only accept the forms provided. Any forms that have been recreated or altered in any way will not be accepted. If you have a suggestion for improvement of a form, please submit your suggestion to Cornerstone Customer Service and we will make every effort to accomodate your request.

Most all Cornerstone forms are provided in PDF format. In order to successfully view the PDFs, you must have the most recent version of Adobe Acrobat Reader. If you need to download or upgrade this software, it is available free of charge on Adobe's web site.

If you have the latest version of Acrobat Reader but still experience problems viewing a PDF, right-click on the link, select "Save Target As...", and save the PDF to your desktop. Then open Adobe Acrobat Reader (from your Start menu), choose "File > Open", and select the recently saved PDF from your desktop.

If you have difficulty printing a PDF, try clicking on the "Print as Image" box in the Print pop-up box.

Options for completing PDF forms include:

  • For editable forms, complete the form electronically by tabbing through each field and typing in the information.
    • If the form requires a signature, click on the signature line to insert the appropriate individual's signature graphic (.JPG, .GIF, etc.).
    • If a signature graphic is not available, click on the "Print Form" button to print the otherwise completed form, and obtain the handwritten signature before submitting.
  • Clicking on the “Print Form” button in the top right corner to print the form, then write in the information by hand. 

Options for submitting completed forms include:

  • For editable forms that do not require submission with payment or other documentation:
    • save the completed PDF file to your desktop or network location, then attach it in an email to cstonesupport@cquest.us.
    • click on the "Submit by E-mail" button (when available) to email the form to cstonesupport@cquest.us (and bypass the need to save the form locally).
  • Fax a hard copy of the completed form to Cornerstone Customer Service at (217) 541-7441 (unless the instructions for a specific form indicate otherwise, in which case an alternate name/fax number will be provided).
  • Mail a hard copy of the completed form to Cornerstone Customer Service, CQuest, 500 S. Ninth Street, Springfield, IL  62701 (unless the instructions for a specific form indicate otherwise, in which case an alternate name/address will be provided).
