Cornerstone User Information
Cornerstone Enhancement Request Form
The Cornerstone Enhancement Request Form can be used by Cornerstone agencies to recommend changes they feel will enhance the Cornerstone system.
For any suggestions, please complete the appropriate section(s) of the Cornerstone Enhancement Request Form and fax it to the Customer Service fax line at (217) 541-7441. Any supporting documentation (screen prints, reports, etc.) should also be submitted with the completed form.
