Cornerstone User Information
EI Provider Information Request Form
The EI Provider Information Request Form is used by Early Intervention providers. Instead of calling the Cornerstone Call Center with questions about multiple claims, EI providers should complete the EI Provider Information Request Form and fax it to the Cornerstone Call Center at (217) 541-7475. The information is then forwarded to the appropriate support group, who will contact the provider with the needed information.
