Cornerstone User Information

Change in Cornerstone Contact Form  

The Change in Cornerstone Contact form is used to report any changes in the Cornerstone system contact names. Cornerstone contacts include:

  • Cornerstone Liaison
  • WIC Coordinator
  • Case Management Coordinator
  • Agency Administrator
  • Cornerstone equipment inventory contact
  • Any other Cornerstone contacts, or users who utilize the Cornerstone Call Center

Information is used to update mailing lists and databases maintained by the Illinois Primary Health Care Association/CQuest America Inc.

Agency personnel should complete all sections of the form, and use the "Comments" section as necessary (i.e., to note that this is a change in the contact person's title, or that this is a new contact person to be added and the old contact name should be deleted).

After completing the Change in Cornerstone Contact form, fax it to the Cornerstone Call Center, Attn: Support Database Specialist, at (217) 541-7475. A staff member from the Call Center will contact the agency to verify the change in information, then update the mailing list and databases accordingly.

Alternatively, you may report changes in Cornerstone System contact personnel information by calling the Cornerstone Call Center at toll-free 1-877-447-4221.
