To begin the MAC process, review the MAC Overview (Word | PDF) then complete the MAC Application Form posted below. Use Form A if you are requesting to open a new site, close an existing site, or moving equipment to a new address. Use Form B if you are requesting to add equipment or move equipment internally (within the same address). For assistance completing the Application Form, please contact the DHS Regional Cornerstone Liaison representing your agency.
For Single Server Solution the MAC process begins my completing either Form SSS-A or SSS-B. Again, for assistance completing the Application Form, please contact the DHS Regional Cornerstone Liaison representing your agency.
If the MAC request is approved by your DHS Regional
Cornerstone Liaison, the CQuest Hardware Services Coordinator will contact you to help you determine which MAC forms are appropriate to your situation/request. The Coordinator will direct you to the menu below in order to access/download the required forms. These forms are available in PDF format or in Microsoft Word format. If desired, you can save the files to your hard drive by right-clicking once on the hyperlinked file name, then using the "Save Target As..." function.
Also please note that in order to participate in any MAC activity, your Agency is required to have completed the most recent annual inventory process. The CQuest Hardware Services Coordinator will notify you if your site has not completed the inventory process. The Agency will then be required to conduct the physical inventory in order to continue with the MAC process.
For best results, right-click on the link, select "Save Target As...", and save the PDF to your desktop. Then open Adobe Acrobat Reader (from your Start menu), choose "File > Open", and select the recently saved PDF from your desktop.
