Important!

The AV form can be completed:

  • electronically by tabbing through each field and typing in the information.  The "Signature of Responsible Agency Official" can be completed by inserting the individual's signature graphic (.JPG, .GIF, etc.) in the field. If a signature graphic is not available, click on the "Print Form" button to print the otherwise completed form, and obtain the handwritten signature before submitting.
  • by clicking on the “Print Form” button in the top right corner to print the form, then writing in the information by hand.

Also please note that in order to participate in the AV program, your Agency is required to have completed the most recent annual inventory process. Hardware Services staff will notify you if your site has not completed the inventory process. The Agency will then be required to conduct the physical inventory in order to continue with the AV process.
